Public Complaint Form

To file a formal complaint, complete and submit the form below.
 
All complaints will be heard in accordance with Policy GB(LOCAL) or any exceptions outlined therein.
 
Informal resolution is encouraged but does not extend any deadlines in Policy GB(LOCAL), except by mutual written consent.
